New Patient Forms
All new patients are asked to print out and complete the new
patient forms
packet.
You can also examine our HIPPA
notice of privacy policies.
If you are unable to fill out these forms ahead of time, please come to
your first appointment 15 minutes early to finish the necessary
paperwork.
Recommendations
before and after the treatment
When you first arrive, please have a seat and make yourself at home in
the waiting room. I will come get you when it is time for your
appointment. It is suggested that you have a small meal within a couple
of hours before your treatment (or at least a snack on the way to the
clinic). Also, please refrain from intense physical activity, alcohol
or caffeine for about 4 hours after your treatment. Drinking plenty of
water after your treatment is also recommended.
What you should wear
It is recommended that you wear something comfortable and loose
fitting (i.e. sweatpants, a comfortable T-shirt, shorts etc.)
that can be easily pulled up to the elbows and knees. Please refrain
from wearing perfumes/colognes, and try to avoid eating or drinking
anything that stains the tongue before your visit. Typically you will
be asked to remove bracelets and watches, so you may want to keep
jewelry to a minimum.
Appointment length
Each visit is an hour long. During the initial visit we will review
your medical history in depth, discuss your main reasons for coming to
acupuncture and come up with a treatment plan, followed by a short
initial treatment. Subsequent visits will include a brief recheck
discussion and a longer acupuncture treatment.
Payment
Cash, personal checks, Visa, Mastercard and Discover credit cards are
all accepted methods of payment. In addition, Medical Flexible Spending
Account (MSA and FSA) flex-cards are also welcomed.
Health Insurance
Coverage
Some insurance companies are now covering acupuncture sessions! It is
important to check with your policy provider about your specific
coverage. Payment is required at the time that services are
provided. However, if you have insurance that covers acupuncture, you
may be given a special payment receipt (called a Super Bill) that can
be submitted to your insurance company for reimbursement.
Late Cancellation
Policy
All appointments that are cancelled with less than 24 hour notice or
are missed without notifying the office will be subject to a late
cancellation fee unless previously discussed.
